Health & Wellness
C-Forward, Inc. encourages employees to live a healthy lifestyle. Employees have the option of participating in a Wellness Program. Employees can choose one of the following plans:
- Gym Program-Employees who have taken a health assessment survey, had at least one physical exam per year, had regular dental checkups and work out at least once a week are eligible for gym membership reimbursement up to $40 per month.
- Athletic Program- Employees who have taken a health assessment survey, had at least one physical exam per year, had regular dental checkups and participated in an organized athletic program are eligible for reimbursement of participation fees.
- Healthy Living Program-Employees who have a BMI at or below what is considered to be normal according to national statistics are eligible to receive a quarterly bonus. Those whose BMI is above normal, qualify for the bonus if they lose 10% of their above normal weight.
